You can easily access statement forms here. Before filling out a statement form, you must have already reported the incident to a member of the University of Wisconsin-Platteville Police Department or a UW-Platteville police officer.

Instructions

Theft

  • Make sure you give a detailed account of the incident.
  • Remember to write down the day/date/time you last saw the item(s) and the day/date/time you noticed it missing.
  • Remember to include the place it was last seen or left at. Example: 139 Brockert Hall.
  • Describe the item(s) in detail. Provide a serial number if you have it or contact the officer with it as soon as possible.
  • Did you or did you not give anyone permission to take/borrow the item(s)? Please include that in your statement.
  • Provide the officer with the estimated value of the item(s). That value is not what it would cost to replace it. Rather, it is based on age and condition of the item.

Vandalism

  • Make sure you give a detailed account of the incident.
  • Remember to include the place where the incident occurred. Example: Lot 1.
  • Did you or did you not give anyone permission to vandalize the item(s)? Please include that in your statement.

Other

  • Make sure you give a detailed account of the incident.
  • Include dates and times if you can remember them.
  • Remember to include the place/address where the incident occurred.
