The University of Wisconsin-Platteville wants you to know that you can file a report of sexual violence or sexual harassment. Keep in mind:

  • All appropriate steps will be taken to respect the privacy of the individuals involved, while maintaining the ability of the University to investigate the incident and take appropriate action.
  • We may take interim measures to ensure the safety and educational your rights and the rights of all individuals involved.
  • The university will take steps to protect you from retaliation.

You also have the right to report sexual violence to the police, and the university can assist with that process if requested.

File a Report

To file an official report of sexual violence or sexual harassment, click this button and fill out the online form. This is the OFFICIAL reporting form for sexual violence incidents and the only report required of Responsible Employees. Please utilize this form to report instances of sexual violence & sexual harassment if you are required to by nature of your position at the University (Responsible Employee) or you are a victim and wish for contact from the University. Information entered here will be submitted to the University's Title IX Coordinator and the Dean of Students Office.

Mandatory Reporting

Wisconsin State Statute 36.11(22) requires states that "any person employed at an institution who witnesses a sexual assault on campus or receives a report from a student enrolled in the institution that the student has been sexually assaulted shall report to the dean of students of the institution."
