Please follow the checklist below to complete the application for this scholarship.
Any freshman or transfer female who has an appreciation for engineering, mathematics, and science and displays outstanding leadership, excellence in academics and extra-curricular activities. One $500 scholarship will be awarded in Spring for Fall tuition. Students who declare major in the following programs are eligible for this scholarship:
Select a teacher, counselor, or professor that you like (and one who likes you) and ask that person to write a letter of recommendation. Remember that most teachers are very busy so it is important to give them enough time to write a letter. Ask when it may be picked up. Some teachers may prefer to send it to UW-Platteville directly. If the teacher chooses to do so, you may want to check with the teacher at a later date to confirm that the letter was sent.
While waiting for the teacher to write the letter or recommendation, continue to work on the application. Make sure that the form is filled in completely. Sections A, B, and D must be completed by the applicant. Applications which are not filled in completely will be disqualified. For freshman applicants, Section C must be filled out by a high school counselor. Transfer student applicants should include their most recent transcript with the application packet and need not complete Section C of the application form.
Write a one-page statement describing your commitment to your chosen major, your personal goals and your financial need. Your statement should also show how you meet the criteria of the scholarship. Please also attach a one-two page resume. If you do not have a resume, please use the one-two pages to detail your extra curricular activities, honors and leadership experiences.
(Freshman applicants only): Section C of the application must be completed by the applicant's High School Counselor. In order to save the counselor time, it is suggested that a large brown envelope is pre-addressed so that the application and supporting materials are ready to mail. Transfer students must send a copy of the transcripts form the most recent educational institution attended and do not need to fill out Section C. Bring all materials, including the letter of recommendation, the application, other supporting materials to the counselor. Verify, with the counselor, that all of the requested information is included and that you have all required signatures. The applicant's ACT scores must be included. Once all the material is completed and assembled, staple the packet together.
The full application and all supporting materials must be received by the Women in EMS Program by March 1, 2013 in order to be considered for this scholarship. The application and accompanying materials should be mailed to the address noted above.
