At least one of the two Recommendation Forms must be filled out by one of your professors. Academic advisors and TA's are also acceptable. The second form may also be completed by a faculty member/advisor or by a current or former employer.
You can and should fax your forms to our office (608.342.1736) if it is close to the application deadline. However; we require original signed documents for your application to be complete for review.
While we prefer you use our UW-Platteville official application forms, we have made exceptions to permit use of a student's home school's application forms. Please check with us to make sure your home school's application forms are sufficient.
No, the application fee is not refundable.
Yes, the application fee and the confirmation deposit are built into the overall program fee and will be deducted from the listed program price on your bill.
Yes, you can submit all parts of the application without paying the application fee; however, your file will not be reviewed until the application fee is paid.
