| Attached are the guidelines, application cover page and letter of support form for the 2009 UW – Platteville Academic Staff Award for Excellence. This award recognizes an individual academic staff member who has made distinguished contributions to the mission of the university. | |
| Instructional and non-instructional academic staff members that hold appointments of 0.50 FTE or more and have a minimum of three years of continuous employment at UWP are eligible to be nominated or apply. | |
| Individuals holding faculty status, a faculty title or a classified title are not eligible for this award. | |
| Nominations may come from a college, department or division. In addition, instructional and non-instructional academic staff, administrators and faculty may nominate individuals. Self nominations are also welcome. | |
| The recipient of this award will receive a $1,000 honorarium and a plaque. They will be presented at the Chancellor’s Convocation in the fall. | |
| An original plus five copies of the nomination materials should be submitted to Donna Gavin, 222 Ullrich, by the deadline. | |
