With the delivery of Student Self-Service, current students can now view transcripts online. Consequently, the Registrar's Office will no longer print unofficial transcripts. (Click on the Academic Tools link at the top of this page and choose PeopleSoft SA Site to log in.)
Former students and graduates, who would like a copy of their transcripts, please see the instructions below for how to obtain an "official" copy of your transcript.
To obtain your official University of Wisconsin Platteville transcript, you must submit a signed, written request to:
Due to the Family Rights and Privacy Act of 1974 (FERPA), your signature is required for the release of a transcript.
Requests should be placed well in advance of date needed to insure adequate time for processing.
Transcript request forms are available at the Registrar's Office or you can download the form and send your request to the Registrar's Office.
Using the "Transcript Request Form" will ensure that your request is processed as quickly as possible. However, if you are unable to download the form, please make sure your request is legible and includes the following:
An official transcript is printed on security paper and bears the signature of the Registrar and the school seal. Only official transcripts are mailed.
If paying by credit card, you must provide the account # and expiration date.
If you are making payment by credit card and you have access to a fax machine, you may fax us your request**. Again, using the "Transcript Request Form" will ensure that your request is processed as quickly as possible. However, if you are unable to download the form, please make sure your request is legible, and includes all the required information as well as a phone number where we can contact you if necessary.
**NOTE: Transcripts will be sent via first-class U.S. mail service. Please allow adequate time for delivery.
