It is critical to keep the University informed of your current address. If you don't, you may miss receiving important information.

To change your address, complete a "Change of Address Form", available at the Registrar's Office or you can download it here: change of address form.

Return completed forms to the Registrar's Office, 101 Brigham Hall.

OR

Send written notification to the Registrar's Office providing the following:

• full name
• last 4 digits of social security number or student identification number
• new address
• new phone number
• indicate if address is local, home, or other (billing, parent, guardian, spouse)
