LOST / STOLEN STATEMENT FORM

UWS 18.08 Misuse of Parking Services Information

Please carefully read the following and then click on the form link below.

There are several steps in reporting a lost or stolen parking permit.

If you locate your permit DO NOT USE IT.  Return the lost/stolen permit to the Police Department.  A citation (misuse of parking services) will be issued to those using/displaying a lot/stolen permit.

The information below can be printed and kept to fill out in case you locate your lost or stolen permit in someone's vehicle.

You may look for your permit hanging from another vehicle mirror.  If you locate your permit do not approach the owner.    CALL THE POLICE IMMEDIATELY (608-342-1584) and speak to someone.  Do not leave a message.  Take down the following information:

  • Vehicle plate number
  • Make
  • Model
  • Color
  • Lot they are parked in
  • Take a photo of the vehicle if you are able.

If the lost or stolen permit is displayed in your vehicle you will receive a citation for misuse of parking services and all privileges to park on university property will be revoked.
