Do not fill in the “Gender” and “Ethnic” columns unless you know them. Those will be completed by the Affirmative Action Office based on Self-ID forms returned by the applicants.
Fill in the Position #, replacing “EDU-4” with the position code of your search.

List all applicants in the order received. Provide name and candidate number, highest degree and field of study, and years/type of relevant experience. If the candidate is not qualified, give the reason(s).
