2006 UW-Platteville Program Certification Documents
October 1, 2006 to June 30, 2009
In a letter dated October 31, 2006, the Division of Affirmative Action in the Office of State Employment Relations (OSER) informed UWP that it had met all criteria for certification of the AA/EEO program and thus was officially certified through June 2009.

  1. AAO Reports Directly to the Chancellor
    The Affirmative Action Officer reports directly to the Chancellor. A copy of the Chancellor’s letter can be found in Attachment A.
  2. Hiring Review Process
    See Attachment B for a copy of the memo written by the Human Resources Director /Affirmative Action Officer concerning the hiring review process. UW-Platteville is using two different checklists to conform to all state agency requirements concerning checklists and logs. The two documents, "Hiring Review Checklist" and "Classified Recruitment Log" help us to conform to all requirements concerning the hiring process. The forms can be seen in 2304 Ullsvick.
  3. Affirmative Action Advisory Committee
    The minutes of the last meeting held May 1, 2006 can be found in Attachment D. The first meeting of this year will be held September 28, 2006 and then they meet approximately once a month throughout the academic year. See Attachment E for the announcement of the first meeting.
  4. Survey of Employee Disability/Severe Disability Status
    Employees are told of their right to request reasonable accommodations during their orientation process, and at least once every two years all employees are asked to self-identify as persons with disabilities. If any employee self-identifies as a person with disabilities, we take several steps. We ensure that the accommodation needs of the employee are addressed and we ensure that the necessary information is entered into the payroll system using the correct codes.
  5. The Five Essential AA/EEO Policies and Procedures
    There is one change to the Affirmative Action/Equal Employment Opportunity Policy Statement. The phrase “gender identity or expression;” has been added to those protected. A revised copy is attached as Attachment F. The other policies (Reasonable Accommodations and ADA Policy and Procedures,; Policy and Procedures Regarding Discrimination and Harassment [note that this document combines the sexual harassment and discrimination policy and procedures], and the Balanced Interview Panel Policy) have not changed.
  6. Dissemination of AA/EEO Policies and Procedures
    All new employees have received copies of the policies and procedures and all postings are still accurate and in place.
