Complete the form below by entering the chapter name, instructor name, and the names of students participating in any events.  Your chapter will only be registered for events where at least one name appears in that event.  Click on "submit registeration" button at bottom when form is completed.  If you accidently submit your registration (by pressing Enter instead of Tab), you can continue and resubmit the form.  The last form received will replace any previously entered forms.
