RST File Checklist for Calendar Year 2009
Please label the spine of the RST binder with the name of the faculty member, the department, and the request for action (promotion, tenure, retention, merit, and/ or inequity). Thickness of binder may not exceed approximately 1”; binder should include section dividers.
All forms are available on the college web page.
Front of file (inserted and completed by DRB/SRB)
Faculty RST File Activity Record Affix to left inside cover of binder.
- If applicable: Request for retention or tenure (green form); the original form remains in the file until the tenure decision. Three-hole punch and insert before Section 1 divider.
- If applicable: Request for promotion (blue form). Three-hole punch and insert before Section 1 divider.
Section I: Up-to-date cumulative vita (inserted by faculty member)
- All faculty: there is no form for this section. Please provide the type of current vita that you would use if you were applying for a university faculty position.
Section II: Form 1 “Teaching Faculty Evaluation Report” (Form for 2009 inserted and completed by DRB/SRB)
- Tenured faculty: Include Form 1s from the year under review (2009) and the two preceding years (2008 and 2007).
- Probationary faculty: Form 1s should be cumulative.
- Peer evaluations must be conducted and recorded on Form 1 before the student evaluations are reviewed by the DRB/SRB and recorded on Form 1. Actual student evaluations and numerical ratings are to be removed from the binder and retained by the department chair before the file is sent to the CRSTC.
Section III: Narrative statement for current year and/or vita update form (inserted by faculty member)
Section IV: Past narrative statements and/or past vita update forms (inserted by faculty member)
- Tenured faculty: please limit to last two previous years (2008 and 2007).
- Probationary faculty: please include all statements and/or forms since hire.
Section V: Materials added by others (materials for 2009 inserted and completed by DRB/SRB, Dept. Chair, CRSTC, Dean, Provost; materials from previous years inserted by faculty member)
- Examples of materials to be added: letter from DRB/SRB chair; letter from department chair/school director; letter from tenured faculty evaluating probationary faculty. Letters need to identify strengths and weaknesses of the faculty member.
- Tenured faculty: please include materials from the year under review (2009) and the two preceding years(2008 and 2007).
- Probationary faculty: please include materials from each probationary year.
Section VI: New materials added to file after negative decision (inserted by DRB/SRB and/or others)
Section VII: Ancillary supporting materials (inserted by faculty member)
