Who: 2010-11 High school sophomores, juniors and seniors who passed algebra. A waiting list will be created once camp reaches capacity.

Applications are accepted starting now until June 6, 2011.

Where: University of Wisconsin-Platteville campus

When: Sunday, June 19 -Saturday, June 25, 2011
Registration begins at 3 p.m.
Camp concludes at noon

Cost: Includes instruction by university faculty, counseling services, t-shirt and instructional materials.

Commuter $540: includes above, plus lunches Monday-Friday and two evening meals

Residential $755: includes above, plus breakfast, lunch and supper meals Sunday evening-Saturday breakfast and overnight accommodations and evening activities.

No refunds after June 6, 2011.

2. Official transcript with proof of successfully passing algebra
   (Send copy of grade report or transcript to cont_ed@uwplatt.edu)
3. Return permission and medical consent form
4. Payment for $755 if residential student and $540 if commuter student
